We want to help you gain greater power over your health and well-being. This starts with monthly group appointments to learn about all aspects of diabetes self-management, including diet, exercise, and lifestyle changes. You'll meet regularly—first monthly, then quarterly as your condition improves—with the same group of patients, allowing you to develop meaningful relationships with other diabetics and our educators.
Diabetes care is complex, requiring that many issues—besides the primary focus of glycemic control—be addressed. In the near future, US Diabetes Care will partner with specialist practices to provide Podiatry, Optometry, and Diabetic Rehab services under our roof, simplifying your road to better health.
Podiatric foot care
Diabetes can have a serious impact on your feet—with the most severe cases resulting in amputation—yet podiatry is one of the most overlooked aspects of diabetes management. The American Podiatric Medical Association (APMA) recommends that diabetics see a podiatrist at least once a year to prevent complications.
Opthamologic eye care
Diabetic eye disease can be devastating, leading to severe vision impairment or blindness. But early detection and treatment can reduce the risk of blindness by 95 percent. The National Eye Institute recommends that diabetics undergo a comprehensive eye exam at least once a year.
Diabetic rehabilitation center
Movement and exercise factor into every diabetic treatment plan—but it can be challenging to get started. Our in-house rehabilitation center will focus on helping you build strength, stability, and balance so that you can incorporate regular physical activity into your daily life.
We don't prescribe medications, but we do work with your primary care physician and endocrinologist to ensure you receive the best combination of medicine possible.
Lab and pharmacy partnerships
Blood draws, urinalysis, medications: Diabetes tests and trips to the pharmacy can be overwhelming. So we partner directly with a lab and pharmacy to allow for consistency and accuracy in processing your labs and filling your prescriptions—and to make your life simpler.
